Working in partnership to improve health, well being and social care of the people of Hyndburn; reduce inequalities in health and work towards services designed around local peoples needs.
The Health and Social Care Improvement Group, also known as HASCI for short, is a Working Group of Hyndburn FIRST Ltd., Hyndburns Local Strategic Partnership. As a working group we have prepared the Health and Social Care Improvement Chapter of Hyndburns Community Strategy 2003-2008.
What we are trying to do is to encourage partner organisations to work together to improve health and social care in Hyndburn beyond what existing organisations are already doing.
We are working to achieve five key aims:
1. to reduce the prevalence of coronary heart disease lifestyle risk factors in Barnfield, Central, Springhill and Church wards by 2008.
2. to reduce the rate of teenage conceptions in Hyndburn among under 18’s by 50% by 2010.
3. to work towards equity and address health inequalities in Hyndburn by 2010.
4. to develop new health care facilities in Accrington by 2008.
5. to increase the number of older people supported intensively to live at home in Hyndburn by 30% by 2006.
There are a number or organisations participating in the group, which include:
Hyndburn and Ribble Valley CVS, Sure Start Hyndburn, Hyndburn and Ribble Valley Primary Care Trust, Lancashire County Council - Social Services, Hyndburn’s Community Empowered Network (HyCEN), Neighbourhood Renewal Community Voice (NRCV), the Commission for Public and Patient Health Forum, Citizens Advice Bureau and Hyndburn Borough Council.
To date we have supported a number of projects to help achieve the aims of the group, these include:
Acting as a steering group for the Hyndburn Health Needs Assessment 2004.
The Hyndburn Healthy Heart Initiative is sponsored by the Hyndburn Neighbourhood Renewal Fund.
The Sexual Health Outreach Team (SHOUT) are sponsored by the Hyndburn Neighbourhood Renewal Fund.
